The Scuba Sports Club asks that new members submit four forms:

• A membership application form
• A dive interest questionnaire
• An information form
• A liability waiver

All four forms are included in the "Membership Forms" file linked to below.

Divers must complete a liability waiver and diver information form each year before being allowed on any club dives.

For a family membership, print or copy as many extra copies of pages 3 and 4 of the membership forms (diver information form and waiver) as necessary.

Non-divers must complete our non-diver liability waiver before participating in TSSC-sponsored events, and we request that they also fill in the applicable parts (contact and insurance information) of the information form found in the member packet.

The questionnaire is optional, but please complete it - it will help us plan the activities that our members want.

The completed forms, with photocopies of your most advanced certification card, should be mailed to the address given on the form.

Upon joining, a club decal and buddy list of our members will be mailed or given to you.

Membership forms are supplied in Adobe's pdf format. This allows you to view and print the sheet with all of its original formatting preserved. Adobe offers free pdf viewing software for most computers.
